To refer a patient to Johnson Audiology for hearing testing or a cochlear implant diagnostic, fax the order to the appropriate office listed below. We will take if from there and will send a notification to you when the appointment is set and send a report after the patient’s appointment.

To Refer a Patient For Hearing Testing
Send a referral for a "hearing diagnostic evaluation" on your office's physician prescription pad to the appropriate fax number listed to the left. In order to file the diagnostic with Medicare, the script must not mention hearing aids but rather have specific wording for a "diagnostic hearing evaluation."
